Computer Aided Risk Analysis

Time: Three Hours Maximum Marks : 70

Note: i) Attempt any five questions.

1. a)

Define the term "HIRA" and what are the various steps involved in it?

"HIRA" शब्द को परिभाषित करें और इसमें शामिल विभिन्न चरण क्या है?

Critically examine the hazards associated with a power press and suggest suitable guards to safeguard the operator at the point of operation.

पॉवर प्रेस से जुड़े खतरों की गंभीरता से जांच करें और संचालन के बिंदु पर ऑपरेटर की सुरक्षा के लिए उपयुक्�� गार्ड का सुझाव दें।

Explain qualitative and quantitative risk assessment. Describe the layers of protection analysis with suitable example.

गुणात्मक और मात्रात्मक जोखिम मूल्यांकन की व्याख्या करें। उपयुक्त उदाहरण सहित सुरक्षा विश्लेषण की परतों का वर्णन करें।

Conduct a HAZOP study for a batch reactor of your choice. Select at least two study nodes. Apply minimum three Guide Words. Consider flow as the parameter. You may assume any other missing data/information.

अपनी पसन्द के बैच रिएक्टर के लिए HAZOP अध्ययन आयोजित करें। कम से कम अध्ययन नोड चुनें। न्यूनतम तीन मार्गदर्शक शब्द लागू करें। प्रवाह को पैरामीटर मानें। आप कोई अन्य गुम डाटा/जानकारी मान सकते हैं।

What is the purpose of job safety analysis? Give the order of priority for selecting a job for JSA.

With suitable examples, explain Inductive and deductive techniques used in risk analysis.

5. a)

Discuss on human error assessment method. What are the techniques used for human error prediction?

मानवीय त्रुटि मूल्यांकन पद्धति पर चर्चा करें। मानवीय त्रुटि पूर्वानुमान के लिए उपयोग की जाने वाली तकनीकें क्या हैं?

Explain any two techniques used for measuring human reliability.

मानवीय विश्वसनीयता को मापने के लिए उपयोग की जाने वाली किन्हीं दो तकनीकों की व्याख्या करें।

What do you mean by Risk and Hazard? Also, discuss major components of "Risk Management".

जोखिम और खतरे से आप क्या समझते है? "जोखिम प्रबंधन" के प्रमुख घटकों पर भी चर्चा करें।

Discuss the principle, symbol and procedure of Event Tree Analysis (ETA). Write its advantage.

ईवेंट ट्री एनालिसिस (ETA) के सिद्धांत, प्रतीक और प्रक्रिया पर चर्चा करें। इसके लाभ लिखिए।

Construct an event tree for a LPG release from a LPG storage tank located in a LPG bottling plant. Assume frequency values for different situations.
